LFC 'Action for Health' and Liverpool City Council
With support from Liverpool City Council the LFC 'Action for Health' Programme has now been extended through to March 2011.
LFC will be managing community initiatives that aim to promote a healthier lifestyle, increase life expectancy and reduce incidences of coronary heart disease, diabetes and cancers. Liverpool City Council is currently transforming the way it works with people to ensure a more personalised approach and this will be integral to these initiatives.
Some highlights from this last year include:
* 49 vascular check sessions have been delivered to date. 926 individuals have received a health check. 342 males and 584 females.
* We have developed a programme aimed at men aged 18-35 years. Delivered at BNENC and ASCC the sessions are facilitated by the Health Trainer linked to LFC by the NHS
* For those aged 50 years and above we have the 'Half Time Score' healthy lifestyle programme with the emphasis on more to play for in the second half!
* A number of community based health events are planned. The first of these was the flagship event held at Liverpool FC Anfield on 10th December 2009. Over 220 people attended. A number of agencies supported the day including Transitional Care Staff, Age Concern, Liverpool Health Trainers, Housing Trusts, local community services etc.
* 566 Primary School children and 240 secondary have completed a physical activity/healthy lifestyles programme delivered by Alternative Curriculum. Youngsters are given tasks to complete and learning objectives based on promoting healthy lifestyle and confidence.
* In partnership with Liverpool City Council Active City the LFC 'Action for Health' Programme funded the distribution of this under 5's activity resource to all primary schools, Children's Centres and nurseries in the City. Launched at The Echo Arena on 19th March the main aim is to support and encourage families to increase activities with under 5's. This is achieved by using song and dance. Phil Neal helped launch the resource.
* PSS manage the Health Trainer Service on behalf of Liverpool PCT. An agreement was reached to provide an LFC 'Action for Health' dedicated Health Trainer.

LFC will also be working in partnership with AFC Liverpool to deliver aspects of this programme. Other key partners will be Liverpool Active City, Liverpool Primary Care Trust, Age Concern, Mersey Care and many local community agencies.
Throughout the LFC 'Action for Health' Programme we have given emphasis to self directed support. This is a real transformation of adult social care in Liverpool. It is about making real changes in people's lives giving more choice, control and improved outcomes for them and their families.

Community health checks, healthy lifestyle events, schools activity programmes, touch screen information kiosks and supporting literature will be delivered in the North Liverpool area. Liverpool University will undertake the programme monitoring and evaluation.
Liverpool City Council is committed to providing community based resources that support people to live healthy, productive lives and these projects will support and enhance that commitment.
